will be involved in activities that, as we have come to expect with many sc=

activities, will require “permission slips” of one sort or
another. Below are links to s=

Diocese of Manchester forms which we may be asking parents to print, fill o=

and submit to Youth Ministry leadership on an as-required/as-applicable bas=
is.
Diocese
of Manchester Permission Slip. This form allows a minor to partic=
ipate
in a specifically identified activity.&nbs=
p;
There is also a section at the bottom of the slip addressed “f=
or
adult chaperones.”
Parent
Permission to Dispense Medication. This form gives permission for sta=
ff to
assist in providing over the counter medication if requested and to assist =
with
prescription specifically listed on the form. There are two sides/pages to this =
form.
Driver/Vehicle
Registration. This form is used to provide infor=
mation
regarding drivers and vehicles used for ministry purposes.
